The ADO form The ADO Form is an important tool used by registered nurses to formally document circumstances in which the RN believes an assignment they were given is unsafe.
This form is essential for ensuring and documenting unsafe
and dangerous circumstances. The ADO
documents the situation. The ADO form also helps protect a
nurse’s license and shifts the responsibility for dangerous
situations back to management
